This form is to be completed to request leave for your child during term time and must be completed giving at least 2 weeks notice.
The absence will be recorded as an unauthorised absence on your child's record.
A penalty notice may follow.
If there are exceptional circumstances please email d.lewis@waterlooprimaryschool.co.uk for the attention of the Attendance Governor with a minimum of 2 weeks notice for their consideration.
